Cancer Care Services’ program, Stay Abreast – Education to Navigation Connection, two-tiered framework will provide resources in the critical areas of breast cancer resources, education and prevention as well as assist breast cancer patients navigate the cancer care continuum from diagnosis through survivorship.
The program will help remove health care barriers for the target audience, increase their knowledge relative to the importance of routine mammograms, and assist with breast cancer patients’ access to care and services through financial, emotional, spiritual and social support.
